Treatment of Carotid-Cavernou¢¥s Fistula by Embolization with a Muscle block Attached to Fascia.

Abstract
Carotid cavernous fistula has been treated in various ways, including repeated carotid compression, ligation of common or internal carotid artery, trapping procedure, and embolization with muscle, gelfoam and other radiopaque thrombus making materials, Among these the intraluminal embolization method of Brooks¢¥, advocated by Lany and Bucy9), is simple and effective but has not been used widely because the cerebral circulation may be obstructed at the distal arterial segment. The possibility of this complication led many surgeons to clip the internal carotid artery intracranially before the embolization.
This procedure itself was not always simple or without risk. Wanissorn15 advocated use of a large block of muscle about 1-1 times the diameter of the common carotid artery. We also have used a large block of muscle attached to fascia to eliminate chance of its fragments passing into the cerebral circulation and to obtain a better obliteration of the fistula.
We are reporting two cases of carotid-cavernous fistula treated successfully by muscle with fascia embolization.
